SECTION 6. Appointment and Reappointment Requirements. Each person initially appointed

to membership on the Medical Staff of the. Hospital shall be provided with a current copy of
the Regulations of the Hospital and the Medical Staff, Bylaws, Rules and Regulations. Each new

person shall affirm in writing that he/she has read and understands the Bylaws, Rules and
Regulations and agrees to abide by them. Each person who is reappointed to the Medical Staff
of the Hospital shall acknowledge in writing that he/she has read, understands and agrees to
abide by the current Hospital Regulations and Medical Staff Bylaws, Rules and Regulations. The
statement to be signed by each new appointment or reappointment shall include as a minimum:
    (a) An obligation as a member of the Medical Staff to provide care and supervision to
         all patients within the Hospital for whom such Medical Staff member has responsibilities.

     (b) Agreement to abide by the Hospital Regulations and Medical Staff Bylaws, Rules and

          Regulations, and policies and directives that are in force during the time such person

          is a member of the Medical Staff of the Hospital.
     (c) Agreement to accept committee assignments and such other duties and responsibilities

          as may be reasonably assigned. by the Supervisors or the Medical Staff.


     Submission of such signed statement to the Chief Administrative Officer of the Medical Staff
shall be necessary before an appointment or reappointment is effective.


     SECTION 7. Hospital Clinical Board.
     7.1 Composition. The Hospital Clinical Board shall be composed of the Chiefs of Clinical

          Services, the Chairman of the Department of Community Medicine, the Dean and
          Associate Dean for Clinical Affairs of the College of Medicine and the Hospital Director.
          The Deans of the other colleges of the Medical Center, the Director of the University

          Student Health Service, the Associate Dean for Veterans Affairs, the Director of Nursing

          Services and the Vice President for the Medical. Center shall be ex-officio non-voting
          participants. The Dean of the College of Medicine shall serve as Chairman of the Hospital
          Clinical Board.

     7.2 Function. The Hospital Clinical Board acting for the Medical Staff shall establish a

          framework for governance and a means of accountability to the Council of Supervisors.

          It will function as the medico-administrative unit for the Medical Staff. It shall concern

          itself primarily with the quality of care within the Hospital and report its activities
