Instructions for the Secretary:
August 1, 193~

Fill out the two Copies of the Blank Returns, file one
away for future use and mail the other in the addressed
envelope enclosed herewith.

The pages may be separated and typewritten. Use
only a good, clean, permanent record ribbon and send the
original, NOT A CARBON COPY. Otherwise they must
be written in ink, in plain legible writing. You are urged
to use great care and accuracy.

The names in each list must be in ALPHABETICAL
ORDER, the given name last. Write given name in full
instead of initials only. Please do not ditto names. Give
slim-c! number or R. F. D. and post office address. Spell
names correctly or the printer (who uses your return for
his copy) will not get the names correct in the Proceed-
ings of the Grand Lodge.

If a Minister. put “Rev.” after his name; if a Master
Mason affiliated for fifty years, put “fifty years” after
his name, and in margin give name and number of lodge
in which he was made a Master Mason, and if in asylum,
put “in asylum” after his name.

Remember the military report at dress parade: “All
present or accounted for.” So all who were reported as
members last year must be “present” members, “or ac-
counted for” as dead, demitted, suspended or expelled.
All wflb are new members are “accounted for” as having
been members per last return, or who have become mem-
bers byinitiation, admission or reinstatement.

Make out return, check it off by last year’s Return
and prove it—see page one; make a copy in ink or type-
' writer, compare with the original; preserve one copy
and mail the other to the Grand Secretary on or before
September 3rd. The fiscal year closes at low twelve
August 31st.

Include only actual work done during the year, leav»
ing spaces blank where petition, Entered Apprentice or
Fellow Craft Degree was reported in previous return.
Do not include work done as a courtesy to other lodges,
except as a foot note at end of your own work.

Grand Lodge Dues To Be Paid: $1.00 on every mem-
ber whose name is in the list of members in good stand-
ing at low twelve August 31st, without reference to the
date of initiation, reinstatement or affiliation; $1.00 to
the Widows and Orphans’ Home, 50 cents to the Masonic
Home Journal and 25 cents to the Old Masons’ Home, a
total of $2.7 5 for each member and $10.00 on each Master

_ Exemptionsszinisters and Rabbis who derive their
support from preaching, and members who have been
affiliated Master Masons of Kentucky lodges for fifty
years, are exempt from payment of the $1.00 Grand
Lodge dues, but must pay the W. & 0. Home assessment
of $1.00, the Old Masons’ Home assessment 25 cents and
the Masonic Home Journal 50 cents. Members confined
in insane hospitals are exempt from paying Grand Lodge
dues and assessments. “Life members” and “Honorary
members” are not exempt from Grand Lodge dues and
asseSsments.

Lodges U. D. are governed by the same rule as char—
tered lodges, except they do not elect officers and do not
have a seal.

Fifty Year Members: Full details giving date and
name of lodge in which he was made a Master Mason
and names of lodges in which he was affiliated should be
furnished the Grand Secretary prior to August 1st. (Reg.
162.)

Penalty for Delinquency: If any lodge shall fail to
comply with any provision of Section 138—139 or 146, it
shall immediatey be charged with a fine of $5.00 to be
due and payable at once, and in addition thereto said
lodge shall be charged one-half of one per cent per month
on all unpaid sum or sums until same is paid. If the fine
and interest aforesaid are not paid by January lst next
thereafter, the Grand Master shall forthwith arrest the
Charter of such delinquent lodge. (Sec. 140, Amended
1934.)

Important: This Return becomes a permanent record
of your lodge in the Grand Secretary’s officeand may
be the only record in existence in the event your lodge
records were lost or destroyed, therefore, it is very im-
portant that each date be correctly entered; take pride
in making it neat and accurate.

Your Cooperation Necessary: The work of our office
during August and September, while the returns are be-
ing received and checked, is extremely heavy and co-
operation on the part of the Secretaries in sending their
returns immediately after August 31st, and also in an-
swering by return mail any letters of inquiry in reference
to corrections, will assist us very much and will be sin-
cerely appreciated.
