RETURN FOR THE YEAR ENDING
JULY 31

Dear Brother Secretary:

This Annual Return becomes a permanent record
of your lodge in the Grand Secretary’s office and may be
the only record in existence in the event your lodge records
are lost or destroyed, therefore, it is very important that
each name and each date be correctly entered, they must be
complete in every detail and mailed to the Grand Secretary
not later than August 10.

FILL OUT THE TWO COPIES OF THE BLANK RE-
TURNS, file one away for future use and mail the original
copy in the addressed envelope enclosed herewith.

THE LEAVES ARE HELD TOGETHER WITH WIRE
STAPLES. They may be separated and typewritten, then placed
in proper order and the wires replaced, otherwise they must
be written in blue or black ink, in plain legible writing. Mail to
the Grand Secretary the original copy, keep the carbon or pencil
copy.

IMPORTANT. The roster of membership in the annual
Proceedings of the Grand Lodge is copied from this Return,
therefore, great care should be taken in its preparation, its
correctness in facts, dates and spelling, means much to
future generations.

PAY FULL POSTAGE ON RETURNS. Returns are rated
first class mail and require 5 cents per ounce postage; if
they weigh over the one ounce 5 cents will not do.

LIST NAMES IN ALPHABETICAL ORDER, the given
name last. WRITE GIVEN NAMES IN FULL INSTEAD OF
INITIALS. Please do not ditto names. Do not use nlck names
or pet names. Enter names of your members on the numbered
lines and street or R. F. D. and post—office address on the
line underneath the name on the same page."

IF A MINISTER, put “Rev." after his‘name; if a Master
Mason affiliated for fifty years, put “Fifty Years” after his
name; if in Asylum, put “In Asylum.”

LODGES U. D. are governed by the same rule as chartered
lodges. except they do not elect officers and do not have a
seal.

FIFTY YEAR MEMBERS: Full details giving date and
name of lodge in which he was made a Master Mason and
names of lodges in which he was affiliated should be furnished
the Grand Secretary prior to July 1. (Reg. 162). They are not
subject to lodge or Grand Lodge dues and assessments. (Reg.
163-163A).

ALL PRESENT OR ACCOUNTED FOR: Remember the
military report at dress parade: “All present or accounted
for.” So all who were reported as members last year must
be present members. or “accounted for” as dead, demitted,
suspended or expelled. All who are now members are “ac-

EXEMPTIONS: Members confined in hospitals for the ___

TIME LIMIT FOR FILING RETURNS AND PAYME
OF DUES AND ASSESSMENTS: Your report will be checl
against the one filed last year, the time allotted to our «
fice to complete this work is very short and limited, so 1
your Return in promptly. It can be mailed during 1»—
month of July but must be mailed not later than Aug

PENALTY FOR DELINQUENCY: If any lodge shall i
