This Annual Return becomes a permanent record of your
e in the Grand Secretary’s office and may be the only

FILL OUT THE TWO COPIES OF THE BLANK RE-

THE LEAVES ARE HELD TOGETHER WITH WIRE

IMPORTANT. The roster of membership in the annual

PAY FULL POSTAGE ON RETURNS. Returns are rated
t class mail and require 10 cents per ounce postage, if
~ weigh over the one ounce 10 cents will not do.

LIST NAMES IN ALPHABETICAL ORDER, the given
e last. WRITE GIVEN NAMES IN FULL INSTEAD OF
lALS. Please no not ditto names Do not use nick names
aet names. Enter names of your members with street or
‘ . D. and post-office address in numbered blocks.

IF A MASTER MASON affiliated for fifty years, put “Fifty
rs" after name; if in Asylum, put “In Asylum”; if in Old
ons’ Home and not listed as fifty-year member, put “Old
ons’ Home."

LODGES U. D. are governed by the same rule as chartered
_es, except they do not elect officers and do not have a seal.

FIFTY-YEAR MEMBERS: Full details giving date and
e of. lodge in which he was made a Master Mason and
. as of lodges in which he was affiliated should be furnished
Grand Secretary prior to June 1. (Reg. 162.) They are not
ject to lodge or Grand Lodge dues and assessments. (Reg.
-163A.)

ALL PRESENT OR ACCOUNTED FOR: Remember the

having been members per last returns or who have become
members by initiation, admission or reinstatement.

INCLUDE ONLY ACTUAL WORK DURING THE YEAR,
leaving spaces blank (under Initiated, Passed and Raised),
where Petition, Entered Apprentice or Fellow Craft degree
was reported in previous Return. Do not include work done
as a courtesy to other lodges, except as a foot note at the end
of your own work.

GRAND LODGE DUES TO BE PAID: $2.00 on every
member whose name is in the list of members in good stand—
ing at low twelve June 30, without reference to the date of
initiation, reinstatement or affiliation; $4.25 to the Widows &
Orphans’ Home, $1.00 to the Masonic Home Journal, and $2.25
to The Old Masons’ Home, a total of $9.50 for each member;
$1.00 for each member raised for the George Washington Ma-
sonic National Memorial and $5.00 for each candidate initiated.
(Sec. 131, Sec. 142, Reg. 19, SR. 1-16, Amended 1970.)

EXEMPTIONS: Members confined in hospitals for the in-
sane, Old Masons’ Home, honorary members and members
awarded 50-year buttons are exempt from lodge and Grand
Lodge dues and assessments. (Sec. 143-143A.) Life and dual
members are not exempt from Grand Lodge dues and assess-
ments.

TIME LIMIT FOR FILING RETURNS AND PAYMENT
OF DUES AND ASSESSMENTS: Your report will be checked
against the one filed last year, the time allotted to our office
to complete this work is very short and limited, so get your
Return in promptly. It can be mailed during the month of June
but must be mailed not later than July 31. (Sec. 146.) Your
remittance must reach the Grand Secretary’s office not later
than September 1 (Sec. 139, Amended 1970.)

PENALTY FOR DELINQUENCY: If any lodge shall fall
to comply with any provision of Section 138, 139 or 146 of this
Constitution, it shall in each case, be assessed a fine of $1.00
per day for delinquency, and the Grand Secretary will collect
the same from the lodge in the same manner as the collection
of Grand Lodge dues, and if any lodge persists in its delin-
quency, the Grand Master is authorized in his discretion to
arrest its charter for failure to comply with the law or pay
the fines. (1971.)

YOUR COOPERATION NECESSARY: The work in our
office during June, July, August and September, while the
returns are being received and checked, is extremely heavy and
cooperation on your part in forwarding returns promptly and
answering by return mail any letters of inquiry with refer-
ence to corrections to complete your report, will be greatly
appreciated.
